We offer complex lung examination:
- spirometry (basic curve F-V) including bronchodilatation tests
- body plethysmography with the lung diffusion testing
- impulse oscillometry
- non-specific bronchoconstriction tests with metacholine in children and adults
- bronchoprovocation stress tests with running outside, or on the bicycle ergometer
- ergooxytensiometry
- spiroergometry
Body plethysmography (whole body plethysmography)
- examination allows measurement of intra-thoracic gas volume (non-directly measurable static lung volume and capacity) and measurement of respiratory tract resistance
- basic spirometry is a part of the examination as well, curve flow-volume
Advantages of the examination:
- exact determination of the ventilation disorder and its level (evaluation of the restrictive ventilation disorder with the knowledge of TLC)
- assessment of the pulmonary hyperinflation according to the RV and RV%TLC
- bronchodilatation tests with the assessment of the reversibility of airway obstruction
Contraindication is non-cooperation of the examined person during breathing examinations.
Pulmonary diffusion examination – measuring of the transfer factor TLCO
Is indicated when there is a suspicion of damage of the lung parenchyma or disorder of lung circulation, damage of the function of alveolar-capillary membrane.
Recommendation: bring the results of hemoglobin, do not smoke before the examination.
Contraindication is non-cooperation of the examined person during breathing examinations.
Impulse oscillometry in children and adults
- it is used to measure respiratory tract resistance (higher with obstructions of respiratory tract)
- this examination is indicated for small children (2 years and older), or for older and non-cooperating patients
- examination is independent on cooperation and only needs calm ventilation through mouthpiece while sitting
- it is also possible to complement bronchodilatatic test for evaluation of the dynamics of the respiratory tract resistance of the central and peripheral respiratory tract.
Non-specific bronchoconstriction tests with metacholine in children and adults
- assessment of bronchial hyperactivity (BHR)
Indication:
- dif.dg. of coughing, emphysema, suspicion for the bronchial asthma
- monitoring of the BHR during the treatment of bronchial asthma (activation of the disease during treatment)
- assessment purposes
Absolute contraindication:
- in adults, acute upper airway disease meaning FEV1 under 1,2 l
- heart-attack less than 3 months before the test
- stroke less than 3 months before the test
- uncontrolled hypertension SBP over 200mmHg, DBP over 100mmHg
- known arterial aneurysm
- inability of the examined to understand the test and cooperate
Relative contraindication:
- exacerbation of asthma, infectious airway disease (2-6 weeks interval)
- pregnancy and breast feeding
- epilepsy during medication treatment
- myasthenia gravis
- fever and infectious diseases
Warning for recommending doctors- prior to the examination (BPT) it is necessary to stop using these medicaments:
- short-term working beta2- mimetics 8 hours before the test
- long-term working beta2- mimetics 48 hours before the test
- methylxanthine derivates 8- 24 - 48 hours before the test (according to the medicine)
- antihistamines 4 days before the test
- chromoglycans 8- 48 hours before the test
- stop using inhaled corticosteroids 1-2 months before the test
- antileukotrienes 24 hours before the test
Do not stop using medicaments only if we examine protective effect of the medicaments.
Examined person (legal representative of children) signs informed agreement with the examination at our department.
We recommend: do not drink coffee, chocolate, cola, black tea and do not smoke before the test, empty stomach is not necessary.
Bronchoprovocation stress tests for children and adults
- to exclude post-stress bronchospasm
Possibilities of testing:
- children can run outside on a flat surface, child is accompanied by a nurse and parent, it runs for about 6 minutes with control of the pulse and O2 saturation during stress
- adults are tested on bicycle ergometer for about 8-10 minutes
- assessment of the ventilation parameters is done before and after stress
We recommend: the day before examination should be without – heavier physical activity, do not drink coffee before the test, eat moderately, do not smoke, or drink alcohol 12 hours before. Use regular medication. Take appropriate clothes and shoes, towel, drinks for the test, it is possible to have a shower after the test at our department.
Examined person (legal representative of children) signs informed agreement with the examination.
Spiroergometry
- Complex stress test on the bicycle ergometer connected with ventilation examination, analysis of exhaled gases, monitoring of BP, ECG, pulse oxymetry and eventually collection of blood gases and parameters ABR (from arterialized blood from the earlobe).
Indication:
- assessment of stress tolerance and possible limiting factors
- differentiation of cardiac and pulmonary emphysema
- exclusion of post-stress bronchospasm
- detection of limitation of blood gases exchange
- cardiovascular or ventilatory limitation
- assessment of the level of the damage in chronic pulmonary diseases
- assessment of physical (working) fitness
- assessment purposes
- pre-operation functional examination before pectoral (as well as other) surgeries
Absolute contraindication:
- acute heart disease (AIM, acute heart failure, myocarditis, pericarditis, thrombophlebitis, non-stabile AP, serious arrhythmia, tight aortic and mitral stenosis, hypertrophic cardiomyopathy, acute pulmonary embolism)
- malignant hypertension SBP above 240 mm Hg and DBP above 120 mm Hg
- active chronic liver, kidney, thyroid gland diseases
- global respiratory insufficiency, metabolic disruptions
Relative contraindications:
- heart failure NYHA I-III, less severe rhythm disorders
- severe musculoskeletal disorders and serious mental disorders
- unwillingness or disability of the patient to cooperate

Ergooxytensiometry:
- stress test on a bicycle ergometer with measuring of oxygenation of blood, non-invasively with pulse oxymeter and an exclusion of latent respiratory insufficiency.
Examined person signs informed agreement with the examination at our department.
We recommend: the examined person should not do any heavy physical activity, eat moderately before the examination, should not drink coffee, should not smoke and drink alcohol 12 hours before the examination. Keep regular medication. Take sport clothes, shoes, towel, drinks, it is possible to take shower at our department.
